1159 Add address grouping for a business associate.

Add the capability of specifying, for a Business_associate, a group of addresses with a role (e.g., "West coast office", "residence").

Add new entity and new relationships

Add new entity Ref_address_qualifier. Add new "be qualified by" relationship from Address to Ref_address_qualifier. Add new "be specified for" relationship from Address to Facility. Add new "be located at" relationship from Phone_number to Physical_address. Add new rule on Address to say that at least one of business_associate or facility must be specified.

Modify uniquess constraint and relationship aggregate.

Modify attribute ref_phone_number in Phone_number to be a SET and modify its description accordingly. Modify the uniquess constraint on Phone_number to remove ref_phone_number. Modify the uniquess constraint on all subtypes of Address to add facility. Modify diagram EPB to add new relatinships and new entity.
